Empathy Interview Consent Form

In order to better the classroom environment and improve the school district’s processes, Human Restoration Project (a nonprofit organization) is conducting a series of conversations with students and community members. In these sessions, students at your child’s school will be leading a discussion about what’s going well – and what challenges they are facing – in their academics in their school district. This includes questions such as…

Basics

Your student’s name will be anonymous within the transcript with identification based on their grade level (e.g. “7th Grade Student”). Their voice (audio) can be anonymized upon request. Parts or all of the transcript may be shared with the school district.

Benefits

Participating may mean that their opinions and ideas are used to inform policy and practice. Stakeholders will hopefully learn about, and feel more connected to, other people in their school/community through this conversation.

Risks

We do not anticipate any risks in this conversation. Students may pause, stop, or leave a conversation at any time. Participation is voluntary and conversations center on typical school activities and engagement.

How will it be used?

Consent means we can use the conversation audio and transcription for research and professional development purposes.

Data will be stored on Polaris Education, our recording platform, according to our privacy policy: https://www.polariseducation.ai/privacy. There are no financial incentives for participation and no data is sold, trained on, or marketed to third party companies. Anyone can choose not to answer any question and can leave at any time.

If you have any questions, would like to opt out of this consent agreement at any time, or if you would like to request your conversation remain private or be deleted, please email support@polariseducation.ai.

Opt-Out

If you do not wish for your child to participate in these focus group/empathy interview conversations, please email support@polariseducation.ai and/or contact your school administrator.
